Online Registration for NEW STUDENTS- Registration will open on July 22, 2024! Click HERE on July 22 to begin.

Once registration is complete, submit the following items for your new student to the school office:

• Student's certified birth certificate

• 3 documents for proof of residency. Click here for residency requirements.

• Kindergarten Medical Requirements

Medical forms including:

• Eye Exam (Grade K)

• Dental Exam (Grades K, 2, 6)

• Illinois Physical Exam  (Grades K & 6)

• Sports Physical Exam Form

• Medication Authorization Forms

• Action Plans for Medical Conditions:

  • Asthma

  • Food Allergies

  • Seizure

• Kindergarten students must be 5 years old by September 1, 2024 to be eligible for the 2024-2025

  school year.

Online Registration for RETURNING STUDENTS will be July 22-29, 2024! Click HERE on July 22 to begin.

• This includes students that attended Central last year for early childhood, speech services and kindergarten students that have already completed pre-registration in Skyward.

• Log into Skyward Family Access between July 22-29, 2024 to complete online registration for your student!

Central Primary School Requests for labeling supplies:

• Kindergarten- Teachers will communicate how they would like school supplies to be labeled

• 1st-3rd Grade- Label all school supplies with first name and initial of last name.
